چکیده:
مقدمه؛ سکته مغزی با اختلال در عملکرد فیزیکی و روانی به عنوان یکی از ناتوان کننده ترین بیماری های نورولوژیکی در بزرگسالان می باشد. بنابراین، یافتن درمان جدیدی جهت افزایش توانبخشی روانشناختی مبتلایان بعد از سکته مغزی مهم است. پژوهش حاضر با هدف تعیین اثربخشی تحریک مکرر مغناطیسی فراقشری مغز بر افسردگی و انطباق اجتماعی بیماران مبتلا به سکته مغزی انجام شده است. روش کار: این پژهش، نیمه آزمایشی با طرح پیش آزمون و پس آزمون با گروه کنترل نابرابر بود. جامعه مورد مطالعه آن کلیه بیماران مبتلا به سکته مغزی مراجعه کننده به کلینیک های مغز و اعصاب شهر تهران در سه ماهه اول سال 1398 بود که همگی آنها دارای تروما از ناحیه پیشانی مغزی بودند. بر این اساس 30 نفر از بیماران با دامنه سنی بین 25 الی 55 سال و بر اساس ملاک های ورود و خروج به مطالعه و با استفاده از روش نمونه گیری غیر تصادفی (هدفمند) انتخاب و 15 نفر در گروه آزمایش و 15 نفر در گروه کنترل جایگزینی شدند. برای اندازه گیری متغیرهای پژوهش از آزمون افسردگی بک، مقیاس خودارزیابی انطباق اجتماعی بوسک و دستگاه تحریک مکرر مغناطیسی فراقشری (Magstim ؛ ساخت کشور انگلیس) استفاده شد. گروه آزمایش 30 جلسه 20 دقیقه ای تحت درمان تحریک مکرر مغناطیسی فراقشری بر روی قشر پشتی جانبی پیش پیشانی نیمکره چپ قرار گرفته و سپس از هر دو گروه کنترل و آزمایش پس آزمون صورت گرفت. داده ها با نرم افزار spss نسخه 21 تجزیه و تحلیل شدند. یافتهها: نتایج تحلیل واریانس چندمتغیری نشان داد که روش تحریک مکرر مغناطیسی مغز در ناحیه ی پشتی جانبی قشر پیش پیشانی نیم کرهی چپ، با فرکانس 20 هرتز در بهبود یافتن نشانه و علایم افسردگی و همچنین ارتقا عملکرد و انطباق اجتماعی بیماران مبتلا به سکته مغزی موثر است (01/0> p). نتیجه گیری؛ با توجه به نتایج این پژوهش، تحریک مکرر مغناطیسی مغز برخلاف داروها که دارای عوارض شـناختی هسـتند باعث بهبود عملکرد و انطباق شناختی و در نهایت افسردگی می شـود. بنابراین پیشنهاد می گردد از نتایج پژوهش های انجام شده در این زمینه به صورت عملی در مراکز توانبخشی بیماران سکته مغزی استفاده شود.
Objective: Stroke is the second leading cause of death in the world and is considered as one of the most debilitating neurological diseases in adults and is more prevalent in low- and middle-income countries. On average, 1 to 10 percent of people who have a stroke die from the disease, and 60 to 70 percent of those with stroke can live independently, but this life is accompanied by physical disabilities. And psychological will be long. Therefore, it is important to find a new treatment to increase the psychological rehabilitation of patients after stroke. The purpose of this study was to determine the effectiveness of Repetitive Transcranial Magnetic Stimulation on treatment on depression and Social adaptation in patients with stroke.
Method: The present research was a quasi-experimental design with pre-test and post-test with unequal control group. The study population was all stroke patients referred to Tehran Brain and Cognitive Clinic in the first trimester of 2019, all of whom had trauma to the forehead. Based on this, 30 patients ranging in age from 25 to 55 years were selected based on inclusion and exclusion criteria and using purposive sampling method and 15 patients in the experimental group (rTMS) and 15 patients in the control group (waiting list) were replaced. Ethical considerations: All subjects received information in the study, at any time, could leave the study. They were assured that all information would remain confidential and would only be used for research purposes. For privacy reasons, the subjectschr('39') details were not recorded. In the end, all of them received informed consent. So Beck Depression Inventory, & Busque Social Adaptation Self-Assessment Scale and Repetitive Transcranial Magnetic Stimulation device (Magstim; Made in England) were used to measure the variables of the study. The experimental group underwent 30 sessions of 20 minutes of repetitive extra cortical magnetic stimulation on the lateral dorsal cortex of the left hemisphere of the left hemisphere, areas 9 and 46 of Broadman at a frequency of 20 Hz, 2.5 seconds of stimulation and 10 seconds interval between each stimulation. Then post-test was performed from both control and experimental groups. Data were analyzed by SPSS 21 software.
Results: The findings of multivariate analysis of variance show that the method of repeated magnetic stimulation of the brain in the lateral dorsal region of the left prefrontal cortex, with a frequency of 20 Hz, is effective in improving the signs and symptoms of depression as well as improving performance and social adaptation in patients with stroke. Conclusion: According to the results of this study, repeated magnetic stimulation of the brain, unlike drugs that have cognitive side effects, improves cognitive function and adaptation, and ultimately depression. Also, stimulation with higher frequency and intensity of stimulation increases the response to treatment in patients. He has a stroke. Therefore, it is suggested that the results of research conducted in this field be used in practice in rehabilitation centers for stroke patients.
