Abstract:
مقدمه:این مطالعه با هدف " طراحی، اجرا و ارزشیابی بسته جامع مراقبت معنوی در خودمراقبتی و معنای زندگی بیماران تحت همودیالیز" انجام شد.روش کار: مطالعه ترکیبی-متوالی- اکتشافی حاضر در سال 1399 -1400 در جامعه بیماران تحت همودیالیز مراجعه کننده به بیمارستانهای وابسته به دانشگاه علوم پزشکی تهران انجام شد. ابتدا با 18 بیمار تحت همودیالیز با نمونه گیری هدفمند، مصاحبه های عمیق، نیمه ساختارمند انجام شده و سپس مبتنی براین مصاحبه ها نیازهای بیماران استخراج شد. در فاز کمی 60 بیمار دارای معیارهای ورود وارد مطالعه شدند. مداخله هر هفته به مدت 8 هفته، مداخله انجام شد. نتایج: یافته های بخش کیفی شامل دو طبقه اصلی فنا شدن با زیر طبقات اضطراب مرگ، بدبینی به زندگی، درد معنوی و احساس رهاشدگی بود. دومین زیر طبقه تاب اوری با زیر طبقات حلقه وصال با ذات الهی، تاملی بر آفرینش، هدف آفرینی و هماهنگی روح بود. در بخش کمی نیز عملکرد خود مراقبتی با میانگین ( 00/19) 86/120) و معنای زندگی با میانگین ((45/10)06/40 ) پس از مداخله در بیماران تحت همودیالیز افزایش یافته بود.نتیجه گیری: نتایج حاصل از مطالعه حاضر نشان داد، با وجود اینکه افکار منفی و اضطراب مرگ و فنا شدن در بیماران همودیالیز وجود دارد، اما معنویت و اعتقادات معنوی می تواند، کمک مؤثری در پذیرش بیماری و تکیه گاه مناسبی جهت ارتقاء سلامت جسمی، روانی و اجتماعی بیماران همودیالیز باشد.
Introduction: The aim of this study was to design, implement and evaluate a comprehensive spiritual care package in self-care and the meaning of life for hemodialysis patients.Methods: A mixed-sequential-exploratory study was performed in 2020-2021 in the population of hemodialysis patients referred to hospitals affiliated to Tehran University of Medical Sciences, Tehran, Iran. First, in-depth, semi-structured interviews were performed with 18 patients undergoing hemodialysis with purposive sampling, and then based on these interviews, the patients' needs were extracted. In the quantitative phase, 60 patients with inclusion criteria were included in the study. Intervention The intervention was performed every week for 8 weeks. Results: The findings of the qualitative section included the two main categories of mortality with the subcategories of death anxiety, pessimism about life, spiritual pain, and feelings of abandonment. The second subclass of resilience with the subclasses of the joiner ring with the divine essence was a reflection on creation, purpose-making, and the harmony of the soul. In the quantitative ward, self-care performance with an average of 120.86 (19.00) and meaning of life with an average of 40.06 (10.45) after intervention in hemodialysis patients had increased.Conclusions: The results of this study showed that although there are negative thoughts and anxiety of death and annihilation in hemodialysis patients, but spirituality and spiritual beliefs can be an effective aid in accepting the disease and a good support for promoting physical, mental and social health in Hemodialysis patients..
