Abstract:
هدف: جایگاه بخش خدمات و به ویژه خدمات سلامت در اقتصاد کشور به طور پیوسته در حال ارتقاء بوده است. در این میان سهم حوزههای مختلف عملکردی سلامت تاکنون مورد غفلت واقع شده است. با توجه به عدم حضور جدی بیمههای اجتماعی و بیمههای خصوصی، این حساسیتها در بخش توانبخشی بیش از سایر حوزههای عملکرد بهداشت و درمان بوده است. در این میان اجرای طرح تحول نظام سلامت و تزریق منابع جدید به این بخش، حساسیتهای پیرامون چگونگی هزینهکرد این منابع در نظام سلامت افزایش یافته است. از این روی مطالعه حاضر سعی دارد تا جایگاه فعالیتهای توانبخشی در نظام سلامت ایران را مورد ارزیابی قرار دهد.
روش بررسی: مطالعه حاضر از روش تحلیل روند دادههای طولی سود برده است. برای این منظور دادههای مربوط به بخشهای مختلف حسابهای ملی سلامت در طول سالهای 1381 تا 1396 استخراج و تحلیل شده است. حسابهای ملی سلامت که در قالب جداول داده-ستانده تهیه و گزارش میشوند این امکان را فراهم میکنند تا سهم منابع مختلف تامین مالی برای کارکردهای مختلف نظام سلامت نیز مشخص شود. بر این اساس سهم فعالیتهای توانبخشی در اقتصاد ایران و منابع تامین مالی این فعالیتها در سالهای مختلف محاسبه و با وضعیت کشورهای مختلف در جهان مقایسه شده است. اطلاعات سایر کشورها نیز از دادههای حسابهای ملی سلامت سازمان بهداشت جهانی و نیز پایگاه دادههای اقتصادی کشورهای عضو سازمان همکاری اقتصادی و توسعه گردآوری شده است.
یافتهها: هزینههای مربوط به خدمات توانبخشی در ایران از 884 میلیارد ریال در سال 1381 به بیش از 2967 میلیارد ریال در سال 1396 رسیده که معادل 0.02درصد تولید ناخالص داخلی ایران در آن سال است. سهم هزینههای توانبخشی از کل هزینههای سلامت در سال 1386 در بالاترین حد خود (3.07 درصد) بوده و در سالهای بعد از آن، همواره روند کاهشی داشته و در سال 1396 به حدود 0.22 درصد رسیده است که کمترین مقدار در طول 16 سال گذشته میباشد. مقایسه سهم اقتصادی توانبخشی از فعالیتهای اقتصادی کشور با کشورهای مختلف نشان میدهد که جایگاه این بخش به هیچ وجه قابل قیاس با کشورهای توسعه یافته نبوده و حتی از بسیاری از کشورهای در حال توسعه همرده نیز پایینتر است. کشورهای تونس، تونگا و مولداوی وضعیت مشابه اقتصاد ایران داشته و نسبتی در حدود 0.05 تا 0.1 درصد از کل فعالیتهای اقتصادیشان را فعالیتهای توانبخشی شکل داده است. مقایسه وضعیت شیوع معلولیت در این کشورها با ایران نشان میدهد که کشورهای مذکور از شیوع کمتری نسبت به ایران برخوردار بودهاند. بررسی سهم منابع مختلف نشان میدهد که پرداخت از جیب خانوارها با 37.6 درصد، بیشترین و دولت با 18.7 درصد کمترین نقش را در مشارکت در تامین مالی خدمات توانبخشی قرار داشتهاند. آنچه شایسته توجه است آنکه این رقم قبل از اجرای طرح تحول نظام سلامت در حدود 65 درصد بوده است. بیمههای اجتماعی در سال 1396 نیز تنها 24.6 درصد از هزینههای توانبخشی را تقبل کردهاند.
نتیجهگیری: بحث عدم دسترسی افراد دارای معلولیت به خدمات بهداشتی و درمانی بحث بسیار جدی در تمام کشورهای جهان است. از هزینههای استفاده از خدمات توانبخشی، همواره به عنوان یکی از عوامل محدود کننده دسترسی به این خدمات یاد شده است. با اجرای طرح تحول نظام سلامت، از سهم هزینههای توانبخشی در کل هزینههای سلامت کاسته شده است. در حال حاضر تامین مالی خدمات توانبخشی به شدت وابسته به پرداخت از جیب خانوارها بوده و منابع مالی بیمهها و موسسات غیر انتفاعی و شرکتها نقش قابل توجهی در این میان نداشته است. از این روی هر زمان که دولت از هزینههای مربوط به این حوزه کاسته است، بار مالی تامین این هزینهها مستقیماً بر دوش افراد و خانوارها افتاده است. از این روی انتظار میرود دولت قبل از اینکه از بار مسئولیت و هزینههای خود در قبال خدمات توانبخشی بکاهد، سایر منابع جایگزین برای تامین مالی این فعالیتها را تدارک دیده باشد. بنابراین توسعه منابع مالی برای ارائه خدمات توانبخشی را باید یکی از مهمترین نیازهای نظام سلامت در ایران دانست.
Objectives: The role of the service sector in general and healthcare services in particular have been promoting in Iran’s economy. Despite that, the share of various functions of the health sector has not been discovered until now. Due to the lack of serious social insurance and private insurance, these sensitivities in the rehabilitation sector are higher than in other areas. In the meantime, the implementation of the Health System Transformation Plan and the injection of new financial resources into this sector have raised concerns about the health system function. So, here is the first attempt to estimate and evaluate the share of rehabilitation services in the Iranian economic and health systems.
Materials and Methods: The study utilized longitudinal trend analysis using the National Health Accounts data during the period 2002-2015. National Health Accounts, which are produced and reported in the form of input-output tables, breaks down the share of different sources of financing for different functions of the health system. According to this method, both the share of rehabilitation services in Iran’s economy and the financing sources of these activities have been calculated and is compared is compared to other countries. Data on Iranchr(chr(chr('39')39chr('39'))39chr(chr('39')39chr('39')))s National Health Accounts has been collected from the Statistical Center of Iran. International data is collected from the World Health Organizationchr(chr(chr('39')39chr('39'))39chr(chr('39')39chr('39')))s National Health Accounts and the databases of the Organization for Economic Cooperation and Development member countries.
Results: Rehabilitation services expenditures in Iran reached to 69.8 million US Dollar (1USD=42500 IRR) which accounts for 0.02 percent of its Gross Domestic Product (GDP). The share of rehabilitation expenditures in total health expenditures was at its highest level (3.07%) in 2007 and has declined in the following years. This share reached about 0.22% in 2017, which was the lowest rate in 16 years. Comparing the share of rehabilitation in Iranchr(chr(chr('39')39chr('39'))39chr(chr('39')39chr('39')))s economy with different countries shows that the prominence of this sector in Iran’s GDP is very low compared not only to developed countries, but also to other developing countries. Tunisia, Tonga and Moldova are similar countries to Iran, in terms of the share of rehabilitation activities in their economy; but there is a lower prevalence of disability in these countries than in Iran. Breaking down the financial resources of rehabilitation expenditure reveals that households’ out –of -pocket payments (37.6%) and government (17.7%) had highest and lowest financial contribution for the activities, respectively. What seems truly noteworthy is that the government funds for rehabilitation services were about 65 percent before the implementation of the Health System Transformation Plan. These figures are more noticeable when we see that, more than 26.7% of total health spending in Iran was borne by the government in 2015.
Conclusion: Inaccessibility of people with disabilities to healthcare services is a very serious issue in the world. The rehabilitation services expenditures have always been mentioned as one of the most important barriers of accessing to these services. Following the implementation of the Health Transformation Plan, the share of rehabilitation activities in Iran’s health market has been shrunk. Currently, rehabilitation services in Iran are heavily dependent on household out -of- pocket payments, and insurances (social and private insurances), nonprofit organizations and private businesses have not significant contribution in the financing of these activities. So whenever the government cuts spending on rehabilitation, the burden of these costs falls directly on individuals and households. Regarding that the rehabilitation activities are astoundingly depended on direct out -of -pocket payments, public authorities should organize a stable financial resource before shirking their indispensable responsibilities. Therefore, development of financial resources for rehabilitation services should be considered as one of the most important needs of the health system in Iran.
