Abstract:
زمینه و هدف: سواد سلامت یکی از مهمترین تعیین کننده های اجتماعی سلامت سالمندان است. مطالعه حاضر با هدف تعیین اثربخشی مداخله آموزشی بر دانش، نگرش و رفتارهای تغذیهای سالمندان با سواد سلامت کافی و سواد سلامت ناکافی، انجام شد.
روش بررسی: یک مطالعه تجربی با مشارکت 192 نفر از افراد 60 سال و بالاتر شهر ایلام از اردیبهشت تا شهریور 1395 اجرا شد. افراد مورد مطالعه در چهار گروه 48 نفره شامل دو گروه مداخله با سواد سلامت کافی و سواد سلامت ناکافی و همینطور دو گروه کنترل با سواد سلامت کافی و سواد سلامت ناکافی تقسیم شدند. برنامه آموزش تغذیه سالمندان شامل چهار جلسه سخنرانی همراه با پرسش و پاسخ، به همراه کتابچه راهنمای تغذیه سالمندان برای گروه مداخله اجرا شد. سطح سواد سلامت افراد مورد مطالعه با استفاده از آزمون سواد سلامت عملکردی بزرگسالان ارزیابی شد. همچنین سطوح دانش، نگرش و عملکرد تغذیهای با استفاده از پرسشنامه محقق ساخته در دو مرحله قبل و دو ماه بعد از مداخله آموزشی ارزیابی شد. برای تحلیل داده ها از نرم افزار SPSS نسخه 21 استفاده شد. آمار توصیفی و آزمونهای کای اسکور، تی زوجی و آزمون دقیق فیشر برای تحلیل داده ها استفاده شد.
یافته ها: میانگین سنی مردان مورد مطالعه 38/2±24/61 و میانگین سنی زنان 45/2±35/61 سال بود. اغلب افراد با سواد سلامت کافی مرد بودند (37/84%)، در حالی که نسبت مردان و زنان در افراد با سواد سلامت ناکافی تقریبا یکسان بود. همچنین همه شرکت کنندگان با سواد سلامت کافی و 58/89% افراد با سواد سلامت ناکافی متاهل بودند. پس از مداخله آموزشی میانگین نمرات دانش، نگرش و رفتار در گروه مداخله با سواد سلامت کافی بطور معنی داری افزایش یافت، به ترتیب؛ (013/0p)، (015/0p) و (001/0p). در گروه مداخله با سواد سلامت ناکافی، فقط نمره دانش پس از مداخله افزایش معنیداری را نشان داد (035/0p). با اینحال تغییر معنیداری در نمرات دانش، نگرش و عملکرد تغذیه ای هیچ کدام از گروههای کنترل صورت نگرفت، (05/0p>).
نتیجهگیری: بر اساس نتایج مطالعه، اثربخشی مداخله آموزشی در زمینه تغذیه سالمندان بطور معنی داری تحت تاثیر سطح سواد سلامت آنها بود.
Introduction & Objective: Health literacy is one of the most important social determinants of the elderly health. This study was aimed to determine the effectiveness of educational intervention on knowledge, attitude and nutritional behaviors of the elderly with adequate health literacy and inadequate health literacy.
Materials and Methods: An experimental study was conducted with the participation of 192 people 60 years and older in Ilam from May to September 2016. The elderly were assigned into four groups (48 subjects in each group) includes two intervention groups with adequate and inadequate health literacy as well as two control groups with adequate and inadequate health literacy. The elderly nutrition education program consisted of four lecture sessions with questions and answers, along with an elderly nutrition manual for the intervention group. The level of health literacy among the elderly was assessed using the Test of Functional Health Literacy in Adults. Also, the level of knowledge, attitude and nutritional behaviors was assessed using a researcher-made questionnaire in two stages before and two months after the educational intervention. The SPSS software version 21 was used for data analysis. Descriptive statistics and Chi-square, paired t-test and Fisherchr('39')s exact test were used for data analysis.
Results: The mean age of the men was 61.24±2.38 years and the mean age of women was 61.35±2.45 years. The majority of men with adequate health literacy were men (84.37%), while the proportion of men and women in those with inadequate health literacy was almost the same. Also, all participants with adequate health literacy and 89.58% of those with inadequate health literacy were married.
After the educational intervention, the mean scores of knowledge, attitude and behavior in the intervention group with adequate health literacy increased significantly (p≤0.013), (p≤0.015) and (p 0.05).
Conclusion: According to the results of the study, the effectiveness of educational intervention in relation to nutrition of the elderly is significantly influenced by the level of health literacy.
