Abstract:
اهداف سالمندی یکی از مراحل حساس رشد انسان، دارای ویژگیها و شرایط خاص خود است، در این میان اضطراب مرگ و کیفیت خواب از شایعترین موضوعات سلامت روان سالمندان است، که با مداخله مناسب میتوان خودمراقبتی و کیفیت خواب در سالمندان را بهبود بخشید. لذا مطالعه حاضر با هدف تعیین اثربخشی رویکرد خودشفابخشی بر اضطراب مرگ و کیفیت خواب سالمندان زن مقیم آسایشگاه صادقیه شهر أصفهان صورت پذیرفت. مواد و روشها: طرح پژوهش نیمه آزمایشی از نوع پیشآزمون، پسآزمون و پیگیری با گروه کنترل بود. جامعهی آماری پژوهش شامل زنان سالمند ما بین سنین ۶۰ تا ۷۰ سال شهر اصفهان مقیم آسایشگاه سالمندان صادقیه در سال ۱۳۹۹ بودند که طی فراخوان عمومی در آسایشگاه سالمندان شبانهروزی، تعداد ۴۰ نفر از زنان سالمند داوطلب از طریق نمونهگیری دردسترس و رعایت ملاکهای ورود و خروج انتخاب و به صورت تصادفی ساده در دو گروه ۲۰ نفری گمارش شدند. شرکت کنندگان با شاخصهای اضطراب مرگ کالت لستر و کیفیت خواب پترزبورگ مورد ارزیابی قرار گرفتند. گروه آزمایش تحت مداخله ۱۲ جلسهای خودشفابخشی و گروه کنترل در لیست انتظار بدون دریافت مداخله قرار گرفتند. در پایان برای هر دو گروه مجددا سنجش انجام گردید و پس از گذشت ۴۵ روز مرحله پیگیری بهعمل آمد. و دادههای حاصل از پیشآزمون، پسآزمون و پیگیری از طریق تحلیل کوواریانس چند متغیره (مانکوا) و با استفاده از نرمافزار SPSS نسخه ۲۴ تجزیه و تحلیل شد. یافتهها: نتایج نشان داد که آموزش خودشفابخشی مولفههای اضطراب مرگ (شامل مرگ خود، مردن خود، مرگ دیگران و مردن دیگران) و تعدادی از زیرمقیاسهای متغیر کیفیت خواب )شامل کیفیت ذهنی، تاخیر در به خواب رفتن، اختلالات خواب و مصرف داروهای خواب آور) سالمندان را در شرکتکنندگان گروه آزمایش در مقایسه با گروه کنترل به طور معناداری ارتقاء بخشیده است (۰/۰۵>p) و این اثرات به میزان قابل توجهی در مرحله پیگیری در مولفههای مردن خود، کیفیت ذهنی، تاخیر در به خواب رفتن، اختلالات خواب و مصرف داروهای خوابآور ثابت باقی ماندهاند (۰/۰۵>p). ولی در مابقی زیرمقیاسهای اضطراب مرگ و کیفیت خواب تفاوت معناداری در مرحله پسآزمون و پیگیری یافت نشد (p>۰.۰۵). نتیجهگیری: نتایج پژوهش حاضر نشان داد آموزش خودشفابخشی در جهت تقویت نقش خود فرد موجب کاهش اضطراب مرگ ادراک شده و بهبود کیفیت خواب سالمندان زن مقیم آسایشگاه میشود و به نظر میرسد نمایانگر افقهای جدیدی در مداخلات روانشناختی است و میتوان از آن به عنوان یک روش آموزشی مؤثر در بهبود سلامت روانی و سبک زندگی سالمندان مقیم آسایشگاه سود جست.
Objectives: aging refers to one of the most critical stages of human development, with its own characteristics and conditions. In the meantime, death anxiety and sleep quality are among the most common mental health issues in the elderly since that proper intervention can improve self-care and sleep quality in the elderly. Hence, this study was aimed at determination the effectiveness of self-healing approach in sleep quality and death anxiety of elderly women living in Sadeghiye nursing homes in Isfahan city.
Materials and Methods: This was a quasi-experimental research design of the type of pretest, posttest, and follow-up with a control group. The study’s statistical population included our elderly women between the ages of 60 and 70 years old living in a Sadeghiye nursing home in Isfahan city (Iran) in 2020. During the public call in the boarding nursing home, 40 volunteer elderly women were chosen through available sampling and observation of the inclusion and exclusion criteria and they were randomly assigned to two 20-people groups. Participants were assessed by The Collett-Lester Fear of Death Scale (CL-FODS) and Pittsburgh Sleep Quality Index (PSQI). The experimental group underwent a 12-session self-healing intervention while the control group was assigned to a waiting list without receiving any intervention. In the end, both groups were re-measured and the follow-up stage was performed after 45 days. The results of pre-test, post-test, and follow-up were analyzed by multivariate analysis of covariance (MANCOVA) by means of SPSS software version 24.
Findings: The results revealed that self-healing training significantly improves the components of death anxiety (including self-death, self-dying, others-death, and dying of others) and a number of sleep quality variable subscales (including mental quality, delay in falling asleep, sleep disorders and use of sleeping pills) in elderly participated in the experimental group compared to the control group (ppp> 0.05).
Conclusion: The results of the present study showed that self-healing training to strengthen the role of the individual, reduces perceptual anxiety and improves sleep quality of elderly women living in nursing homes. While representing new horizons in psychological interventions, it may be employed as an effective educational method for improving the lifestyle and mental health of the elderly living in the nursing home.
