Abstract:
اهداف: سالمندان مبتلا به اختلالات روانشناختی از گروه در معرض خطر سوءتغذیه می باشند. بررسی وضعیت تغذیه ای و شناسایی عوامل موثر بر بروز آن، از اقدامات پیشگیرانه و کنترل کننده محسوب می شود، از اینرو این مطالعه با هدف تعیین وضعیت تغذیه ای سالمندان و ارتباط بین وضعیت سلامت و میزان ریزمغذیها با وضعیت تغذیه ای سالمندان بستری در بیمارستان روانپزشکی انجام شد.
مواد و روش ها: در یک مطالعه مقطعی، 101 سالمند بستری در بیمارستان روانپزشکی رازی در سال 1397 ، مورد بررسی قرار گرفتند. دادهها در سه بخش پرسشنامه، دریافت نمونه خون و اندازهگیری نمایه توده بدنی(BMI) جمعآوری شد. پرسشنامه شامل سه قسمت اطلاعات دموگرافیک، ابزار سنجش فشرده تغذیه (MNA) و وضعیت سلامت بود. تجزیه و تحلیل دادهها با SPSS-21 و انجام آزمونهای کای اسکوئر، آنووا و رگرسیون لوجستیک انجام شد.
یافته ها: تقریباً سه چهارم افراد وضعیت تغذیه غیر نرمال داشتند. وضعیت تغذیه ای با وضعیت دندان، سطح فعالیت فیزیکی، BMI و ویتامین دی و ب12 ارتباط معنیدار داشت(0/05>p). عامل های پیش بینی کننده فعالیت فیزیکی(OR=8/539 ,CI=1/142-63/850) ، وضعیت دندان OR=23/119 ,CI=1/401-38/788)) و ویتامین ب 12 OR=1/007 ,CI=1/002-1/012)) گزارش شدند. سطح ویتامین دی در 95 درصد از افراد ناکافی بود.
نتیجه گیری: وضعیت تغذیه اکثر سالمندان غیر نرمال بود و درصد قابل توجهی با کمبود ریزمغذی مواجه بودند. عوامل موثر شناسایی شده قابل تعدیل می باشند. بر این اساس غربالگری و مدیریت منظم وضعیت تغذیه باید به عنوان یک رکن مهم در مداخلات درمانی تقویت شود.
Objective: Elderly with psychological disorders are at risk of malnutrition. Investigating the nutritional status and identifying the effective factors are one of the preventive and control proceeding. Therefore, this study aimed to investigate the nutrition status(NS) and examine the relationship of health status and micronutrients level with NS in elder persons.
Methods and Material: in a cross sectional survey, 101 elderly people admitted to Razi Psychiatric Hospital of Tehran in 2018 were selected as the sample. The data were collected with demographics questionnaire, the Mini Nutritional Assessment and the health status, blood sampling and body mass index (BMI). The data were analyzed by the chi-square, Anova test and logistic Regression in SPSS-21.
Results: Approximately three-quarters of the participants had an abnormal NS. The results indicated that the NS of participants had a significant relationship with dental health status, physical activity, the serum levels of vitamin D, vitamin B12 and BMI score (p)OR=1/007 ,CI=1/002-1/012), physical activity (OR=8/539 ,CI=1/142-63/850) and dental status )OR=23/119 ,CI=1/401-38/788). Vitamin D levels were inadequate in 95% of subjects.
Conclusion: The NS of most participants was abnormal and the majority of them were suffering from micronutrient deficiencies. Results identify modifiable factors associated with malnutrition. Accordingly, regular nutritional screening and management should be strengthened as a pillar component of therapeutic interventions.
